Obtaining

Incredibly Harmful Tome is bought at the Keeper of The Book in the basement of a large ruined structure north of the Fallen Forum entrance at the coordinates (-523x 38y 3z). The trade for the item itself can only appear if the player is holding the Completely Harmless Tome in their inventory for the following materials:

• Knowledge's End: A material found on a balcony near a ghast spawner inside of the Magmortic Atheneum POI inside of the largest room.
• A Revelation On Knowledge Itself: A material found inside of The Fallen Forum's Conscriptorium Branch. The book can be found near the entrance into the branch behind a gravel wall in the corner. The coordinates of the material are -1243x, 137y, 8348z inside of The Fallen Forum.
• Wax Seal: A material found inside of the Bastion of Spires POI in the basement surrounded by warped blocks on a bookshelf.

At the Keeper of The Book with Knowledge's End and A Revelation On Knowledge Itself, you can create An Untold Story, and combine it with the Wax Seal to create an Uncompleted Mystery. With the Uncompleted Mystery and Completely Harmless Tome, you can purchase the Incredibly Harmful Tome.
